The following three forms are mandatory for your camper to attend Camp Scully.

• FORM 1 - PHYSICIAN’S EXAM FORM - Not needed for Day Camp. If your child is staying overnight we MUST have this form. A doctor must sign or stamp this form. Please upload this form to your Camp In Touch account. *This form or an Annual Physical is REQUIRED for the Day Camp Overnight Experience. Please fax it or email it as follows:

  FORM 2 - HEALTH HISTORY - Parents must complete a Health History for each child attending camp. This is so important that if you are applying to Overnight Camp, your child will not be enrolled until it has been submitted and reviewed by us. Have your child’s immunization dates handy so you can complete the online form with ease and get it done today.

• FORM 3 - RELEASE AUTHORIZATION - We only release campers to people that you authorize. When completing this form, please remember to include yourself in the list AND to hit the “SUBMIT” button on the bottom.
