The following three forms are mandatory for your camper to attend Camp Scully.

• FORM 1 - PHYSICIAN’S EXAM - Not needed for Day Camp. If your child is staying overnight we MUST have this form. A doctor must sign or stamp this form. Please upload this form to your Camp In Touch account. *This form is REQUIRED for Day Camp Overnight Experience: If your child is doing Day Camp with the one night Overnight Experience, fax it or email it.

  FORM 2 - HEALTH HISTORY - Parents must complete a Health History for each child attending camp. Have your child’s immunization dates handy so you can complete the online form with ease.

• FORM 3 - RELEASE AUTORIZATION - We only release campers to people that you authorize.
